This is the first document Southall supplied to explain what the SC files were. Now then given the level of security alleged in this document (remember Southall has given evidence that the SC files relate to clinical audit now) how have Mrs H's files gone walkabout? You may notice that the North Staffs Logo on this document looks very dodgy indeed.
