CHILD protection officials failed to recognise the danger to a toddler who was tortured and killed by her stepfather, a report said yesterday.
But social services chiefs said lessons will be learned from her death in a bid to ensure it does not happen again.
A report into the death of the two-year-old said there was 'a failure to recognise indicators of risk'.
The review of the case of the child called Lois also found that even when a risk assessment was undertaken it was 'inadequate'.
Officials in charge of social services in Caerphilly county, where Lois lived with her mother and stepfather, said staff training and support had now been improved and procedures strengthened.
Lois, who lived on Caerphilly’s Lansbury Park estate, died on November 8, 2004, from a head injury. She had been placed on the child protection register a month earlier.

Kylie Minogue has joined with NSPCC and ChildLine in support of a new campaign to raise funds for abused children, The Child’s Voice Appeal.
The services received 2.3 million calls in the last year from more than half a million kids needing help.
Many reported calling several times just to get through to a counsellor.
The appeal aims to fund up to 15 extra call centres next year, and boost volunteers by about 600 to help get to every call first time.

The fathers words were almost lost in the lofty ceiling vault. If he expected a final decision, he did not get it. Mr Justice Hedley wearing a suit (no wig or robe) told him he was sending the case to Croydon where the mother lived. This had two advantages: a single district judge could take charge of the case and a contact regime could be set up at a special centre. I would appreciate it, the man said. 'Ive not had any contact with my children since 2006.'
In his room the judge explained that the intractable family dispute had run for years. The man had been offered contact under supervision. He had refused but wanted to reapply.
As the family liaison judge for Greater London Mr Justice Hedley, 62, is responsible for family courts in the capital outside the High Court. But this was holiday time and he was doing a stint of vacation duty a mix of emergency or procedural applications in continuing cases that need attention.
Judges, and the system, feel beleaguered. The Times has accused family courts of operating in a conspiracy of silence particularly in care cases. While injustices may occur, judges feel that the picture is distorted. So The Times had been granted rare access to sit on the bench and witness family justice from a judges-eye view.
Mr Justice Hedley, who with his wife had two children and fostered three others, adopting two of them, admits that he may have handled more childrens cases than any other judge. By the law of averages over 16 years judging he must have made mistakes, he admits. A leading children's charity today called for an end to the "volatile, wasteful and reactive" policymaking towards children and young people which it said had dominated the past 21 years.
Ahead of the new parliamentary year, Action for Children, formerly NCH, said many policies have been driven by political point-scoring and media headlines rather than hard evidence.
Since 1987, there have been more than 400 initiatives, strategies and acts of parliament affecting children and young people in theUK, an expert report published by the charity today revealed. But governments have not allowed proper time to implement the "avalanche" of policies - more than three-quarters of which have been introduced under Labour in the past 10 years, it said.
Frequent changes
As a result, children and social workers are failing to benefit from services that are subjected to frequent changes in policy, funding and structure, the study showed. Many children's projects have "barely enough time to be set up and begin to deliver services before staff have to plan for reconfiguration or even closure due to lack of long-term funding".
Policies have too often been triggered by high-profile tragedies, such as the death of eight-year-old Victoria Climbie in 2000, drawing resources to children at the "hard end" while leaving others in need. I was approached in March this year by two members of the EastEnders research team on behalf of executive producer Diedrick Santer, to say they were thinking of running a grooming storyline and asked if I thought this was the sort of thing that needed to be aired. I said, yes, absolutely. Its a very important issue and of course EastEnders is a hugely popular programme, reaching over nine million homes every week so the message would reach a wide audience.
The writers then gave me the storyline which I was allowed to modify and shortly after that the scripts began arriving. They were very detailed but stuck to the parts which involved the main characters involved in the grooming plot.
There were only two developments which concerned me and one of these involved Whitney the paedophiles target self harming. I asked them to pull that line because although it can often be part of a sex abuse story I felt it needed to be told in its own way and it could have introduced unnecessary complications. I felt it was hard to cover both issues at the same time.
The second area where I asked for changes hinged on Tony- the groomer never having sex with his girlfriend, Bianca. This is not realistic and might be dangerous because it gives the message that paedophiles do not have sex with adults - and thats not true

The PSNI arrested a man in a major inquiry into an online paedophile forum in theUK. Investigators believe they will locate thousands of subscribers to the internet site around the world.
Details of the inquiry came to light following the jailing of the internet forums 'librarian' at Teeside Crown Court in England earlier this week.
Philip Anthony Thompson (27) had 241,000 indecent images of children on computer equipment at his home.
He played a key role in maintaining the forum, which was established as a place where paedophiles could be introduced to one another.
Subscribers were invited to post images and make comments before being subjected to questioning by other users in a bid to discover if they were undercover detectives. Once they passed the vetting process users were directed to other online channels.
The site was discovered by the Metropolitan Polices Paedophile Unit in May last year and the investigation quickly developed into a national and then an international effort involving 12 UK police forces and agencies in 33 countries, including Australia, the US and Canada.
It emerged last night that the PSNI is one of the police forces involved in the inquiry, which is codenamed Operation Elm. It is understood a number of different operations have since been launched in connection with Operation Elm.
The Ulsterman arrested in relation to the inquiry is one of 50 people arrested across the UK so far, including a fire-fighter, a university lecturer and a police community support officer. It is understood the local man is currently on police bail. A PSNI spokesman confirmed: The PSNI has made one arrest in connection with a UK-wide operation against images of child abuse. Police would not reveal any further details about the mans arrest.

A brief history about my abuse. I was abused from the age of five, we believe. One of my brothers began the abuse and added in other ones over the years. There isn't a lot of memories for me and I have not tried to bring anything forward.
My healing began the day I had enough and told my sisters what had been happening to me. I thank God they believed me and confronted my last abuser and it ended. I then started to go to counselling once I realized my life was on a downward spiral. I slept with any guy who wanted me and then I would walk away. Later I came to realize in counselling that I was looking for love and acceptance.
My brother hated me so much, why never mattered. The only time he treated me nice was when he was abusing me. Through therapy I came to understand how messed up my thinking had become. I associated sex with love. It always left me feeling empty inside. Part of my heeling process was to confront my abusers. I was able to confront all of them and some responses were good and some not so good.

Surely now is the time to focus on the reality behind the likes of Families Need Fathers and Fathers 4 Justice, the truth being that the former has been going for over 3 decades without success and apart from annual membership fees are also heavily funded by the very government who has done much to undermine family life since 1997, suffice to say FNF can't run with the hares and hunt with the hounds, for to do so is nothing short of hypocrisy. As for F4J, its founder now blames a father who killed two children as good enough reason to abandon his paid up members for the second time in just a few years, loyalty to his members? I fear not!
This subject is not nor never should be about membership fees, personal egos or a nine till five paid job paid for by the State, its about the here and now its about securing parity in family law this in the best interest of children and parents alike and sadly both organizations sited above have failed abysmally at the expense of others misfortune and for far to long.
We say this therefore, for over three decades concerned organizations have picked up the gauntlet on our behalf most have come and gone disillusioned and exhausted by the apathy of both State and victim. Hence we say this in all honesty the NSCFC can not do this for you, no more than any other charity or organisation out there can as has been proven time and time again. That you might in all good faith continue to pay membership fees to support groups year in year out hoping for their success, was, is and never can bring the powers that be to your rescue, suffice to say dont waste your money on empty promises. If you want change as we your fellow victims do, if you want to see your beloved ones again, if you want to stop this continued erosion of your rights as a father, mother, grandparent or extended family member then the fight is yours to behold and in your hands only. Conclusion being, as with the poll-tax, people power on mass was and is the one and only true option left to retrieve what we and our loved ones have lost, indeed which of these two issues are of more importance, the fight for no poll- tax or the children of your heart? The choice is ours and if we want to secure a better future for those that follow then I submit to you that we must grasp this moment now before it is too late, for without doubt todays child is tomorrows parent and God forbid we should leave them to face the same fate as we.

A single mother meets a man who she likes but it worried that she does not know enough about his background to allow him fully into her family's life. She telephones the local police and requests information about the man.
The police would check the background of the man because the request has come from a mother - someone who is directly responsible for a child. Officers would carry out two checks - a priority check within 24 hours followed by a more thorough risk assessment which takes longer because it will delve into someone's history.
If there is a criminal record, the pilot constabularies say they would use special child protection measures, jointly run by police and probation officers, to work out how best to deal with the suspect.

Families Need Fathers (FNF) is to pay tens of thousands of pounds in damages and legal costs after one of its officials admitted writing a defamatory letter to a judge in an attempt to undermine the fathers custody case.
Steve Stephenson, FNFs London branch organiser, admitted that he should not have made the false and defamatory allegations, which were vetted by the head office of the government-funded charity.
The case has highlighted the secrecy in which the family justice system is shrouded, because the father who cannot be named for legal reasons was unaware of the allegations against him for several months.
It was only when the father, a working professional in his fifties, requested correspondence from his file that it came to his attention and he refuted the allegations. Three court hearings had taken place in the meantime.

But Lancashire County Council admitted the services had 'not moved on' after the OFSTED Joint Area Review (JAR), completed in May, described services for children being looked after as 'inadequate'.
Two council officers quit their jobs ahead of the publication of the report today.
Gill Rigg, director of children's integrated services, and Sue Fadipe, head of central services, left last week. But the council today refused to confirm whether the two were connected.

'I'm not convinced unpaid work will change the way people think or behave,' said Anthony Douglas, chief executive of Cafcass, which advises the courts on the interests of children in family proceedings. 'It might be the right thing to do in a handful of cases. But what we really need is more specialist family support services with counselling for both children and parents.'

Two unnamed doctors surveyed their colleagues at a top London hospital and found three quarters carried unsecured memory sticks with confidential data.
The two medics said the practices were likely to be similar elsewhere.
Hospital doctors routinely carry memory sticks containing names, diagnoses, X-rays and treatment details - but these should be protected by passwords.
The pair who carried out the survey spoke to 105 doctors at their hospital. In total, 92 held memory sticks with 79 of them containing confidential information.

The Independent Safeguarding Authority (ISA) is tasked with better protecting children and vulnerable adults by pooling data from several agencies.
The first 60 employees have moved into the ISA's new base at Stephenson House in Darlington.
About 250 people will work for the ISA by the time it launches in 2009.

Ohio mum China Arnold was accused of killing month-old tot Paris Talley in 2005.
A judge declared a mistrial in February, and the retrial began on August 18.
The jury found the 28-year-old guilty last week of aggravated murder.
The defence had argued that Arnold was drunk when the incident took place and argued she had no motive to kill her child.
However, prosecutors called the crime “heinous” and argued for the death penalty.
After about five hours of deliberations, the jury was unable to decide whether to recommend the death penalty or life in prison.

Common Pleas Judge Mary Wiseman lashed out at 28-year-old China Arnold, who chose to watch her sentencing from a side room on a TV screen.
Judge Wiseman said: "No adjectives exist to adequately describe this heinous atrocity.
"This act is shocking and utterly abhorrent for a civilised society."
The Judge rejected a plea by Arnold's attorneys for a minimum sentence of life in prison with the chance of parole after 25 years.
A jury last week spared Arnold the death penalty when it could not reach a consensus.
